Step 1:
Log in with you LSUA credentials at lsua.erezlife.com or http://lsua.it/LSUACARES. We recommend that you bookmark the link for easy access throughout the semester.
Step 2:
Select Cares: Student of Concern Form on the right side of the screen. All other forms are only applicable to students.
Step 3:
Complete form and press Submit at the bottom of the screen.
