What Is the Questionnaire Activity?
The Questionnaire tool allows instructors to create surveys, evaluations, or research-style instruments directly inside Moodle.
It is ideal for:
Student feedback surveys
Needs assessments
Research instruments (e.g., Likert scales)
Pre-/post-course surveys
Self-reflection check-ins
The Questionnaire is not graded and is designed for collecting structured data.
How to Create a Questionnaire
Turn editing on in your Moodle course
Click Add an activity or resource
Select Questionnaire → Add
Enter a Name and optional Description
Choose response options (e.g., anonymous or identified)
Choose opening and closing dates (if desired)
Save and display
Key Settings to Know
1. Response Options
Type:
Respond many – students may submit multiple times
Respond once – one submission per student
Respondent type:
Full name – identified responses
Anonymous – no identifying information stored
Resume: Allows students to return and finish later
2. Availability
Open date – when students can begin
Close date – final deadline
3. Submission Options
Allow branching (skip logic) if your institution has it enabled
Email notification of responses (optional)
Adding Questions
After creating the shell:
Click Questions
Select Add Question
Choose from:
Check boxes
Essay box
Numeric question
Radio buttons
Rate (Likert scale)
Dropdown
Enter question text and save
Tip: Use Rate (Likert scale) for research tools like the Moral Intensity Scale.
What Faculty Can Do With the Questionnaire

Go to the Questionnaire
Click View All Responses
See a summary or detailed view
✔ Download Data
You can export responses for analysis:
CSV (Excel-compatible)
Excel spreadsheet
Text format
Statistical summaries
✔ Analyze Patterns
The results page provides:
Response counts
Percentages
Question-by-question breakdown
Optional individual response view
✔ Re-use or Duplicate Questionnaires
You can:
Copy a questionnaire to another course
Backup and restore it
Reuse the same instrument each semester
Student Experience
Students will:
Open the Questionnaire
Click Begin questionnaire
Answer questions (all at once or in sections if resume is enabled)
Submit responses
Receive optional confirmation message
Note: Students cannot see other students’ responses.
Best Practices
Use anonymous mode for honest feedback
Use Rate (Likert scale) to mimic formal research tools
Keep questions brief and grouped logically
Set a close date to encourage completion
Export results as CSV for easy analysis
Use the Feedback or Quiz tool instead if you need grades
