Maxient Reporting Guide for Students

To report an Incident, Hazing concern, Student Complaint, or LSUA CARES form you will use Maxient.


How to Access Maxient

  1. Log in to MyLSUA

  2. Click Student (top right of the page, next to your profile)

  3. From the dropdown menu, hover over Maxient

  4. Select the form you wish to complete and submit



Choosing the Correct Report Type


Hazing Reporting

Select Hazing Reporting to report hazing behavior.

Hazing is any reckless, intentional, or negligent act during initiation into or affiliation with a student organization (such as fraternities, athletic teams, or clubs) that humiliates, degrades, abuses, or endangers a student — regardless of their willingness to participate. Hazing may involve physical or psychological harm and is often used to maintain group hierarchy.

Select Incident Reporting to report a Title IX or Student Conduct issue.


LSUA CARES

LSUA Cares Form is an essential student support resource that provides mental health screenings and a dedicated system for reporting students of concern. Through its online questionnaire and reporting tools, LSUA Cares helps identify and assist students who may be experiencing stress, depression, or other challenges that impact their academic success. This form can be submitted by faculty, staff and students.


Student Complaint

Select Student Complaint to report concerns or issues involving interactions between students and LSUA employees.

Before completing this form:

  • Click and carefully read the “Student Complaint Policy” link (located in blue at the top of the form)

  • Ensure you have followed and exhausted all steps outlined in the policy before submitting the complaint

Completing the Form

  • Fill out the form completely and accurately

  • Provide as much detail as possible

  • Upload any supporting documentation (emails, screenshots, photos, files, etc.)

Important: The more complete your report, the faster it can be reviewed and addressed.
